Mild hyperbaric oxygen therapy (mHBOT), which involves breathing pure oxygen in a pressurized chamber, is primarily used as an adjunctive treatment rather than a primary cure. For cancer patients, its benefits center on healing treatment-induced damage and potentially enhancing the efficacy of standard therapies. 

1. Healing Radiation-Induced Damage 

mHBOT is a proven treatment for Late Radiation Tissue Injury (LRTI), which occurs when radiation therapy damages blood vessels and causes tissue or bone death months or years after treatment. 

  • Osteoradionecrosis (ORN): It helps heal radiation-damaged bone, most commonly in the jaw, by stimulating new blood vessel growth (angiogenesis).

  • Soft Tissue Repair: It is effective for radiation-induced damage in the bladder (cystitis), bowel (proctitis), and chest wall, reducing symptoms like bleeding, pain, and swelling.

  • Fibrosis Reduction: Research indicates mHBOT can significantly reduce radiation-induced skin fibrosis and lymphedema. 

2. Sensitizing Tumors to Treatment

Oxygen levels within a tumor significantly impact how well treatments work. mHBOT can address tumor hypoxia (low-oxygen areas), which often makes cancer cells resistant to traditional therapies. 

  • Radiosensitization: Oxygen-rich cancer cells are more vulnerable to radiation. Using mHBOT immediately before radiation may improve local tumor control.

  • Chemosensitization: By improving blood flow and oxygenation, mHBOT may enhance the penetration and toxic effect of certain chemotherapy drugs on cancer cells.

  • Immunotherapy Support: Emerging research suggests mHBOT might reprogram the immunosuppressive tumor microenvironment, potentially boosting the effectiveness of PD-1/PD-L1 antibody therapies. 

3. Supportive Care and Recovery

  • Wound Healing: It is often used before and after surgery to accelerate healing, particularly for skin grafts or flaps at risk of failure.

  • Infection Control: High oxygen levels boost white blood cell activity, helping the body fight bacteria and prevent infections.

  • Symptom Relief: Some patients report improvements in cancer-related fatigue and "chemo brain" (cognitive fog) following treatment sessions. 

4. Safety and Considerations

  • Not a Cure: There is no strong scientific evidence that mHBOT cures cancer on its own.

  • Growth Concerns: While earlier theories feared oxygen might "fuel" cancer growth, modern systematic reviews conclude that mHBOT does not promote tumor progression, recurrence, or metastasis.

  • Contraindications: mHBOT should not be used with certain chemotherapy drugs (e.g., doxorubicin, bleomycin) as it may increase their toxicity. Always consult an oncologist before beginning treatment.
